Client must have had at least 72 hours of sobriety, or be detoxed for a length of time appropriate to the substance being abused. Complete Admission Form (Admission Form must have the referral source and addictions and treatment history completed) and Medical Form. Fax or mail the completed forms to Action North (Contact Us). Once the above has been completed, Action North will fax or email a Letter of Tentative Acceptance. Letter of Confirmation of Payment needs to be completed before client arrives at the Centre. The main Treatment Program is 28 days long. The Follow-up Program is 14 days long. The Long-term Program is 90 days in length. Payment Requirements: Out of Province and non-funded residents $150/day Alberta residents $40/day Program costs may be covered by the Alberta Government or other agencies. All steps mentioned above need to be completed five (5) days prior to Admission Date. No exceptions will be made. If above steps have not been completed by this date, client will be moved to next available intake date. Intake is every two weeks on Monday. Once a client is accepted they should check in once a week. Client must arrive at the centre by noonTuesday on the week of Intake, or have a pre-approved altrnate arrival time. If not, they will lose their booking and be moved to the next Intake Date.
